Here’s how to install it:
Just throw the .sbsar file on the shelf and change the type to Generator.

How to use:
Add a generator slot to the desired layer and then drag the generator onto the slot. Adjust the sliders to suit your needs.
I’ve provided the Substance Painter project file so you can see it in action.
